Job Title: Registered Nurse (RN) – Residential Care
Location: Horizon Manor North and South Upper Montclair NJ
Job Type: Part-Time
Shifts: Monday- Wednesday-Friday :11:00am to 4:00pm

The Role
We are seeking a compassionate and clinically skilled Registered Nurse to oversee the health, wellness, and care plans for our residents. You will collaborate with our interdisciplinary team including staffing to ensure the highest standards of safety, compliance, and compassionate care at our two facilities.
Key Responsibilities
Resident Assessment: Conduct initial evaluations, monthly reviews, and ongoing physical/mental monitoring of residents, including medication management.
Care Plan Execution: Administer medications, treatments, and therapies in accordance with the New Jersey Nurse Practice Act and physicians' orders.
Team Leadership: Supervise and delegate daily care tasks with facility providers and CNAs, ensuring facility and state regulatory compliance.
Family Communication: Build meaningful connections with residents and their families, updating them on changes in conditions and care goals.
Documentation: Maintain accurate, timely charting using our medical records and including annual MDS assessments.
Requirements
Active, unencumbered Registered Nurse (RN) license in the State of New Jersey.
Graduate of an accredited Registered Nurse program.
[e.g., Minimum of 2 years] of clinical experience, preferably in residential care facility, assisted living, or sub-acute care.
Current CPR certification plus
Strong critical thinking skills, time management, and a genuine passion for senior care.
